A patient and carer-based evaluation of our new 'Integrated Care' programme
The Integrated Care programme is a major piece of health and social care transformational work that aims to support those most at risk of being admitted to hospital. The programme aims to change the way that patients receive care and the way that their care is organised and administered, with a focus on care being more coordinated and tailored to the needs of the individual.
Urban Inclusion, a local voluntary and community sector organisation has been appointed to work with us to undertake and patient and carer-based evaluation of our Integrated Care programme.
Patients and carers who will be involved in this project will be those people who have been offered the new Integrated Care programme. If this is you, you may be contacted about getting involved in this evaluation soon. By hearing the voice of people using these new services, we will be able to work with them to make sure that services are the best that they can be and offer people what they need.
