NHS Tower Hamlets Clinical Commissioning Group (CCG) has awarded its 2013/14 Winter Planning Innovation Bursary to a project from an east London community development charity, Social Action for Health (SAfH). The winning project, which focuses on improving health management within Tower Hamlets, runs for six months from 1 December 2013 as part of a scheme to improve the health and care of vulnerable groups throughout the winter months, when the demand for health services is at its highest.
SAfH is focusing on encouraging patients with long term health conditions, like diabetes, asthma or high blood pressure, to take control of their own health and well-being. This includes helping patients to understand and manage their medical conditions as well as educating them on how best to use the range of health services.
